Peri-Operative Cardiac Events in Patients with Cardiovascular Diseases Undergoing Non-Cardiac Surgery in a Tertiary Care Hospital of Nepal

Abstract

Introduction: Delivering anaesthesia to the patients with cardiovascular disease for noncardiac surgery is very challenging. We present the prevalence of cardiovascular diseases and perioperative cardiac complications in cardiac patients undergoing non-cardiac surgery in a tertiary care centre of Nepal.

Method: This is a prospective observational study, conducted at a tertiary care hospital of Nepal over a period of one year. The patients having cardiovascular diseases undergoing non-cardiac surgery were included in this study. Patients were evaluated and optimized preoperatively. Either general or regional anaesthesia was delivered to the patients. They were monitored during intraoperative period, postoperative period and followed up until leaving hospital or death. The cardiac events (i.e. myocardial infarction, cardiac rhythm and conduction abnormality,hypotension and hypertension) were recorded and analyzed.

Result: Thirty one patients were presented to the hospital with the diagnosed of cardiovascular disease and underwent different types of non-cardiac surgery during the period of one year. Among them 48% were male and 52% were female. Common intra operative complications were hypertension (29.03%), hypotension (19.35%), and bradycardia (12.9%).Ventricular tachycardia (9.67%), pulmonary edema (6.45%) and myocardial infarction (12.9%) were the common post operative complications. Post-operative mortality was found to be 12.9%. The commonest cause of mortality was myocardial infarction (MI).

Conclusion: The prevalence of cardiac patients undergoing non-cardiac surgery is 2.3%. The commonly encountered perioperative complications were hemo dynamic instability, bradycardia, ventricular tachycardia, myocardial infarction and pulmonary edema.
